Admission Requirement
Health and social care practitioners (registered with their respective boards or councils in Hong Kong, with at least 5 years of working experience in the field, and nomination letter from supervisor of the current workplace would be preferrable)
Course Description
This course is co-organized with the Nethersole School of Nursing under Faculty of Medicine of The Chinese University of Hong Kong. It is designed to equip health and social care providers with essential knowledge and practical skills to conduct Advance Care Planning (ACP) within Chinese communities.
Grounded in evidence-based tools and strategies, the programme offers both knowledge-based and skill-based programme delivered by a multidisciplinary team of experts, including lawyers, medical doctors, nurses, and social workers. Students will learn to navigate complex care decisions in serious illness contexts and communicate effectively with patients and families. The programme features a dynamic mix of presentations, group discussions, and simulated role-playing exercises to enhance learning and real-world application.
This course is funded by the Health and Medical Research Fund, the Health Bureau, The Government of the Hong Kong Special Administrative Region.

This course consists of 5 modules with 24 hours.
• Module 1 (3 hours): Understanding the Concept of ACP
• Module 2 (3 hours): Considering Legal and Ethical Issues related to ACP
• Module 3 (3 hours): Exploring End-of-life Care Options
• Module 4 (12 hours): Conducting ACP Conversations
• Module 5 (3 hours): Integrating ACP into Practice
• Module 1 (3 hours): Understanding the Concept of ACP
• Module 2 (3 hours): Considering Legal and Ethical Issues related to ACP

Learning Outcomes
Upon completion of this course, students will be able to:
